**Responsibilities**

The incumbent provides care, treatment and service to Home Health and Hospice patients and their families in their place of residence within thier scope of practice. Functions as a member of the interdisciplinary team.

**Qualifications**

**Experience:**

+ 2 years LVN Experience within the last 5 years

+ Recent experience as an LVN in a Home Health or Hospice Agency- preferred.

**Licensure:**

+ Currently licensed as a LVN in California

+ Current California driver's license, possession of automobile insurance, & access to an operating motor vehicle.

+ Current CPR/BLS card approved through the American Heart Association.

**Pay Range**

$43.12 - $48.64 /hour
