As a Complete Care Specialist, you will call upon your project management experience daily. You will ensure that Banner Plans & Networks is in compliance with Arizona Heatlh Care Cost Containment System (AHCCCS) requirements for children's behavioral health. You will be responsible for 6-8 special populations and/or AHCCCS initiatives. You will be responsible for project management, strategic planning, meeting facilitation, deliverable completion, community engagement, presenting and training, contract and policy review, and more.

**You must reside in the state of Arizona, preferably the central to southern regions, for compliance for this role** . Your work location will be primarily remote with some in community events. POSITION SUMMARY

This position works with Banner University Family Care (B-UFC) and is responsible for the development and maintenance of system of care needs related to Social Determinants of Health for members of the Arizona Health Care Cost Containment System (AHCCCS) Complete Care Program, including those struggling with mental health and or substance abuse challenges. This position engages in the promotion of the integration of physical health, behavioral health and/or Social Determinants of Health. In conjunction with appropriate supports, the incumbent works with providers to ensure coordination services including but not limited to family support services, independent living skills (ILS) and peer and family run organization services to create a community partnership focused on quality and sustainability.

CORE FUNCTIONS

1. In partnership with other internal departments, as appropriate, develops and implements a system of care that provides a continuum of services delivered to meet the needs of members. This includes the oversight of provider contract deliverables within an established timeframe.

2. Identifies development opportunities and partnerships with stakeholders and community program that enhance the support for AHCCCS Complete Care members.

3. Develops and utilizes processes to track and identify individual member and/or system outcomes.

4. In partnership with other departments, oversees the network development and implementation of community programs and processes that ensure the needs of members are met.

5. Attends internal and external meetings, as needed, as a representative for specialized areas within the Complete Care team.

6. Collaborates with providers to support initiatives for members, that align with programs such as the U.S. Department of Health Services, Substance Abuse and Mental Health Services (SAMHSA) and other established program models.

7. Offers training, support, project management and guidance on serving AHCCCS Complete Care Members.

8. Facility and community-based collaboration while remaining within budgetary allowances. Internal customers: B-UFC Executive Management Team and other B-UFC departments, including other Banner Health departments, as needed. External customers: AHCCCS, provider network, stakeholder and community partners, as needed, that align with system initiatives to achieve identified outcomes and meet the needs of members.

9. Develops collaborative protocols with other health plans and stakeholders as needed to support the needs of members.

MINIMUM QUALIFICATIONS

Knowledge, skills and abilities as normally obtained through the completion of a Bachelor’s degree or certification in Social Work or other relevant field related to supportive services and system development.

Located in Arizona. Significant experience, typically gained through four plus years relevant experience. Must be able to analyze and solve problems. Ability to communicate effectively, both orally and in writing. Ability to correspond and collaborate with internal departments and external entities. Experience with developing, planning and implementing short and long-range goals.

Must be proficient on a computer (PC) with Microsoft Office products. Ability to work with databases/programs and applications.

PREFERRED QUALIFICATIONS

Experience in Arizona Medicaid (AHCCCS) environment.

Master’s Degree preferred (or equivalent work-related experience).

Project management experience preferred.

Additional related education and/or experience preferred.
