**Responsibilities**

The Healthy Families America Family Support Worker (HFA FSW) will visit Welcome Baby high risk families postnatally in the home in a manner that supports the mission of Families in Good Health and the philosophy of the program. HFA Program strategies include using home visits to establish a trusting relationship with participants; strengthen the parent-child relationship; improve parenting skills; improve the family's support system; improve the family's problem-solving skills; and serve as an advocate for child and family. The HFA FSW is responsible for supporting families conducting assessments and providing parent education in the areas of postnatal care breastfeeding parent-infant attachment child development home safety kindergarten readiness and other family health related topics; identifying client strengths needs barriers and risks and providing community resources for those needs; and providing and modeling empathetic support and feedback when working with new mothers infants and family members. The HFA FSW will work closely with other HFA staff in utilizing the First 5 LA-developed Family Strengthening Web-based Case Management System for documentation. The HFA FSW will work effectively as part of a multidisciplinary team as well as with a diverse and high risk population. The HFA FSW will follow HFA protocols and other protocols developed by First 5 LA and will complete other duties as assigned.

**Qualifications**

+ Bachelors degree and a minimum of two years experience in health promotion child development and/or home visitation; or work equivalency of minimum of four years experience in health promotion child development and/or home visitation. .

+ Knowledge of family health care and issues affecting the delivery of health care to families.

+ Experience in health promotion infant and child development and/or home visitation and working with diverse communities that reflect the target population.

+ Experience working with families dealing with multiple challenges; an ability to establish trusting relationships; acceptance of individual differences.

+ Ability to maintain boundaries between personal and professional life.

+ Knowledge of family health care and issues affecting the delivery of health care to families.

+ Bilingually proficient in Spanish/English or Khmer/ English.

+ Speak and understand English well.

+ Experience with computer software programs such as Microsoft Office (as well as computer data entry).

**Pay Range**

$24.09 - $28.63 /hour
